1. 결핵의 진단
1) 결핵의 증상
⑴ 뚜렷한 원인 없이 2-3주 이상 기침 등의 호흡기 증상이 있으면
결핵을 의심하고 이에 대한 검사를 시행하여야 합니다.
⑵ 임상 소견상 결핵이 의심되면 결핵의 과거력, 결핵환자의 접촉
여부에 대해서 물어보아야 합니다.
☎ 결핵의 가장 흔한 증상은 기침이며, 그 외 체중감소, 야간발한
(땀), 발열, 객혈 등이 발생합니다.

2) 흉부 X선 검사
⑴ 흉부 X선검사 결핵의 유용한 진단법이지만 흉부 X-선 단독으로
결핵을 진단하지 않습니다.
⑵ 흉부X선 소견상 결핵이 의심되는 병변이 있으면 가능한한 과거에
시행한 흉부 X선 사진과 비교하여 보아야 합니다.
⑶ 흉부 X선 사진에서 결핵이 의심되면 객담결핵균 검사를 실시하여
결핵을 확진하도록 노력해야 합니다.
☎ 폐결핵환자의 1/3까지 비전형적인 흉부X선 소견을 보일 수 있습니
다.
초기에는 종종 폐렴으로 오인되어 항생제 치료를 하게 됩니다.
과거 결핵 앓은 흉터로 현재 결핵이 아닌 비활동성결핵의 진단은
6개월간 흉부X선 사진에 변화가 없으며 객답검사에서 결핵균이
나오지 않인 경우로 합니다.
3) 미생물학적 진단
⑴ 항산균 도말검사
① 폐결핵이 의심되는 환자는 가능하면 객담을 3회 채취하여
항상균 도말 및 배양검사를 시행하여야 합니다.
☎ 세 개 객담 검체로 검사하였을 때
첫 객담에서 83-87%를 검출하고
두 째 객담은 10-12% 더 검출되며
셋째 번 객담은 추가로 3-5%를 검출됩니다.
1차 객담은 즉석 채취로 하면
집에서 객담을 받아 병원으로 가져 올 때는 냉장보관하며,
병원으로 가져올 때는 객담 통을 종이로 감싸서 햇빛에 노출되지
않도록 합니다.
② 항산균 도말검사는 가능한 빨리 시행하고 도말검사 양성결과는
24시간 이내 보고하여야 합니다.
⑵ 항상균 배양검사
① 결핵균 검사를 위해 의뢰된 검체는 항상균 도말 및 배양검사를
동시에 시행하여야 합니다.
② 검체는 고체배지와 액체배지에 각각 접종하여야 합니다.
③ 항산균 증식이 확인되면 신속하게 결핵균과 비결핵항산균 감별
검사를 시행하여 결과를 통고해야 합니다.
⑶ 결핵균 핵산증폭검사
① 폐결핵이 의심되거나 도말검사가 음성일 때
핵산증폭검사를 하는 것이 좋습니다.
② 항산균 도말 검사가 양성이지만 비결핵항산구균의 가능성이
있을 때 도말 양성검체에 대해 결핵균 핵산증폭검사를 실시하여
결핵균인지 확인하는 것이 좋습니다.
☎ 최근 객담에서 비결핵항산균이 검출되는 경우가 많으므로
도말양성을 보인 검체에서 결핵균 핵산증폭검사가 음성이면
비결핵항산균을 의심할 수 있습니다.
⑷ 약제 감수성 검사
① 전통적인 방법
- 모든 결핵환자의 첫 배양분리균주에 대한 항결핵제에 대한
약제 감수성검사를 실시하여야 합니다.
- 3개월 이상 치료에도 배양 양성이거나 임상적으로 치료실패가
의심되는 경우에 약제 감수성검사를 다시 시행하여야 합니다.
② 신속내성 검사
- 다제내성 결핵이 의심되는 경우 리팜핀 또는 이소니아지드에
대한 신속내성검사를 시행하는 것이 좋습니다.
- 재 치료 시에도 신속내성검사를 시행하는 것이 좋습니다.
☎ 전통적인 방법에 의한 약제 내성검사는 결핵균 배양부터
1개월 이상 걸리는 단점이 있기 때문입니다.
