15. 저혈당 관리

1) 저혈당의 증상과 징후에 대해 환자는 교육받고 저혈당이 의심될 때
가능하면 자가혈당을 측정하여 확인하도록 하여야 합니다.
2) 저혈당이 발생하였을 때 환자가 의식이 있으면 어떤 형태의
탄수화물이든지 간에 15-20g 의 포도당을 섭취하는 것이 가장 좋은
치료법입니다.
☎ 15g의 포도당은 20분 안에 약 40mg/dl 의 혈당을 올릴 수 있습니다.
3) 저혈당의 회복은 일시적일 수 있으므로 치료 후 15분 째 혈당을
다시 검사하여 재 치료가 필요한지 평가해야 합니다.
치료 후 15분 후 자가혈당 측정을 하여 여전히 70mg/dl 미만의 저혈당이
지속되면 한 번 더 포도당을 섭취하도록 하고
혈당이 정상으로 돌아오면 재발을 막기 위해 식사나 스낵을 먹어야 합니
다.
4) 저혈당에 대한 반복된 치료에도 저혈당이 회복되지 않으면 의사의 진료
를 받도록 합니다.
5) 의식이 없는 중증 저혈당 환자는 즉시 글루카곤을 투여 받고 그 가족이
나 동료는 응급센터에 전화를 해서 도움을 요청해야 하며가능한 한
빨리 의사의 진료를 받아야 합니다.
☎ 저혈당이 잘 생기는 당뇨병의 환자는 가족, 동거인, 기타 환자를
돌보는 관계자들은 글루카곤 킷트를 잘 사용할 수 있도록 교육을
받아야 합니다.
6) 의식이 없는 중등 저혈당환자에게 정맥주사가 가능하다면 포도당 10-25g
(50% 포도당 수액, 20-50ml)을 1-3분에 걸쳐 투여해야 합니다.
7) 저혈당 불감증이 있거나 중증 저혈당이 한번 이상 반복되는 경우에는
저혈당의 재발을 막고 저혈당 불감증을 부분적이나마 회복시키도록
하기 위하여 최소 몇 주 동안이라도 환자의 혈당목표를 올리는 것이
적절합니다.
⚇ 저혈당이란 인슐린 또는 인슐린 분비촉진제로 치료받는 사람이
1) 낮은 혈당포도당 농도(혈장혈당 포도당 농도 < 70mg/dl)로 인해
2) 자율신경항진 또는 신경당 결핍 증상이 발생하고
3) 포도당 투여 후에 이런 증상이 소실되는 것으로 정의 할 수 있습니다.
그러나 가끔 혈당치가 정상이라 하더라도 높은 혈당에서 갑자기 떨어질 때 저혈당 증상이 나타날 수도 있습니다.
저혈당이 철저한 혈당조절에 가장 큰 걸림돌입니다.

자율신경항진 증상 신경당결핍 증상
빈맥, 식은땀, 불안감, 오심, 집중이 안됨, 의식혼미, 기력약화
손떨림, 얼굴이 창백해 지는 증상 어지러움, 시력변화, 말하기 힘듬
두통
저혈당 정도에 따른 분류
경 증 : 자율신경항진 증상이 있으며 스스로 대처할 수 있는 상태
중등증 : 자율신경 항진증과 신경당결핍 증상이 있으면서 스스로 대처할 수 있는
상태
중 증 : 다른 사람의 도움이 필요한 상태로 의식이 없어질 수도 있는 상태
대개 혈장 혈당 포조당 농도 < 50mg/dl
⚇ 제2형 당뇨병환자에서 심한 저혈당 발생의 위험인자
① 심한저혈당의 과거력
② 현재 낮은 당화혈색소( < 6.0%)
③ 저혈당 불감증
④ 당뇨병이 긴 유병기간
⑤ 자율신경증 동반
⑥ 낮은 경제 수준
⑦ 청소년
⑧ 스스로 저혈당을 감지하고 치료할 수 없을 정도의 노인환자
⚇ 단순 당질 15-20g 에 해당하는 음식의 예
① 설탕 한 숟가락(15g)
② 꿀 한 숟가락(15ml = 한 큰술)
③ 주스 또는 청량음료 3/4컵(175ml)
④ 요구르트 한 개(65ml)
⑤ 사탕 3-4개
