2. 치료전략
1) 만성 B형간염
약물의 선택은 약물의 효과, 안전성, 내성의 위험도, 비용과 함께
의사의 선호도, 임신계획여부 등을 종합적으로 고려해야 합니다.
라미부딘과 텔미부딘은 장기치료시 내성발현이 높아 선호되지 않습니
다.
클레부딘은 아직 장기간 추적 관찰 데이터가 부족합니다.
아데포비어는 항바이러스 효과가 약하고 1년 이상 사용시 내성 발현이
증가하는 경향이 있습니다.
엔테가비어와 테노포비어는 강력한 항 바이러스효과와 낮은 내성발현
율을 보이는 안전한 약물로 알려져 있습니다.
페그인터페론은 인터페론보다 사용이 편리합니다.
그러므로 만성B형 초 치료는 엔테카비어, 테노포비어, 페그인터페론
중 하나를 선택하는 것을 고려해야 합니다.
◎ 권고사항
면역 관용기는 치료대상이 아니며 만성B형간염에서는 간기능 이상이
있어도 자연적 HBeAg 혈청전환 가능성이 있고, 혈청AST(GOT), ALT(GPT)
가 일시적으로 증가 후 정상으로 안정되는 경우도 있습니다.
경구용 항바이러스제 치료를 받는 환자 중 상당수에서 장기간 투여가
필요하며, 고비용과 약제내성 발현의 문제점이 있으므로 항바이러스제
투여시점을 신중히 결정해야 합니다.
⑴ HBeAg 양성 만성간염
➀ 혈청 HBV DNA ≥ 20,000IU/mL 경우
AST(GOT) 혹은 ALT(GPT)가 정상 상한치의 2배 이상이거나
간생검에서 중등도 이상의 염증 괴사 소견 혹은 문맥주변부 섬유
화 이상의 단계를 보이면 치료를 받아야 합니다.
자연적 HBeAg 혈청 전환 가능성이 있으므로 3-6개월 경과 관찰 후
치료여부를 고려할 수 있습니다.
그러나 황달, 프로스롬빈시간의 연장, 간성 혼수, 복수 등 간부전
이 발생하거나 우려되는 경우 즉각적인 치료를 고려해야 합니다.
➁ 혈청 HBV DNA ≥ 20,000IU/mL이고 AST 혹은 ALT가 정상상한치의
1-2배 사이의 경우, 추적관찰 하거나 또는 간생검을 시행하여
중등도 이상의 염증괴사 소견 혹은 문맥주변부 섬유화 이상의
단계를 보이면 치료를 받아야 합니다.
➂ 초치료 약제는 엔테카비어, 테노포비어, 페그인터페론 알파중
하나를 고려하는 것이 좋습니다.
그 외의 항바이러스제들은 치료에 대한 반응이 좋으리라고 예상되
는 경우 투여를 고려할 수 있으며
치료 반응에 따라 약제의 지속적인 사용 혹은 변경여부를 결정해
야 합니다.
⑵ HBeAg 음성 만성간염
➀ 혈청 HBV DNA ≥ 2,000IU/mL인 경우
AST가 정상 상한치의 2배 이상이거나
간생검에서 중등도 이상의 염증괴사 소견이나 문맥부변부 섬유화
이상의 단계를 보이면 치료를 받아야 합니다.
➁ 혈청 DNA ≥ 2,000IU/mL이고 AST가 정상 상한치의 2배 미만인
경우, 추적관찰하거나 또는 간생검을 시행하여 중등도 이상의
염증괴사 소견 혹은 문맥주변부 섬유화 이상의 단계를 보이면
치료를 받는게 좋습니다.
➂ 초치료 약제로는 엔테카비어, 테노포비어, 페그인터페론 알파 중
우선적으로 고려해야 합니다.
그 외의 항바이러스제들은 치료에 대한 반응이 좋으리라고 예상되
는 경우 투여를 고려할 수 있으며
치료 반응에 따라 약제의 지속적인 사용 혹은 변경 여부를 결정
해야 합니다.
