Ⅲ. 만성 신질환 환자의 신장내과 의뢰 권고안
1. 사구체여과율(GFR) 30mg/min/1.73m²이하인 경우 신장 전문의에게
의뢰하는 것이 좋습니다.
2. 만성신질환 1-3단계는 기본적으로 주치의가 치료를 계속하되
다음의 경우 조기의 신장전문의에 소개, 연계하여 치료방침을 검토해
야 합니다.
① 사구체여과율(GFR) 50mg/min/1.73m²
② 단백뇨 ≥ 1g/일
③ 단백뇨와 혈뇨 모두 양성
④ 빠른 신기능 소실(명확한 이유 없이 4개월 동안 30%이상 감소)
⑤ 저항성 고혈압
⑥ 고캴륨혈증

Ⅳ. 사구체 여과율의 평가
1. 혈청 크레아틴(Scr), 연령, 성, 인종, 체격 등의 변수를 대입하여
사구체여과율(GFR)을 추정하는 방식을 이용합니다.
① 성인: Modification of Diet in Renal Disease(MDRD)공식,
Cockcroft-Gault(CG)공식
② 소아: Schwart공식, Counahan-Barrat공식
2. 검사실에서 혈청크레아틴(Scr)수치를 측정할 때 공식을 이용하여
사구체 여과율도 같이 계산해서 확인해야 합니다.
3. 방정식을 이용하는 것이 아니고 직접 24시간 소변을 수집하여
크레아티닌청소율(Crcl)을 사용하는 경우는 다음과 같습니다.
① 비정상적인 근육량(사지절단, 마비, 근육질환)
② 비정상적인 체격(체질량지수(BMI) < 18.5kg/m²또는 > 30Kg/m²)
로 너무 마르거나 너무 비만한 사람
③ 비정상적인 단백질 식이섭취:
채식주의자, 보디빌더, 크레아티닌 보충제 복용
④ 임신
⑤ 정확한 사구에 여과율 측정이 필요할 때:
신장공여자, 독성이 강한 약물 투여 시
Ⅴ. 단백뇨의 평가
1. 단백뇨를 추적관찰을 위해 단회 요검체를 수집 이용해야 합니다.
2. 단백뇨를 평가하기 위해 24시간 요를 수집할 필요는 없습니다.
3. 아침 첫 소변검사가 선호되나 불가피하면 임의의 시간에 수집한
요검체도 가능합니다.
4. 단백뇨나 알부민뇨 발견을 위한 집단검사 방법은 요 시험지 봉 검사로
충분합니다.
① 요 시험지 봉 검사에서 양성(+1이상)인 경우 3개월 내 정량적 방법
(단백/크레아틴비, 알부민/크레아티닌비)으로 확인해야합니다.
② 지속적 단백뇨는 1주에서 2주사이의 간격을 두고 실시한 3회 이상
의 정량적 검사에서 2회 이상 양성이 나오면 진단할 수 있습니다.
5. 만성신질환 환자에서 단백뇨의 추적 관찰은 정량적 방법으로 합니다.
6. 성인 만성신질환 고위험군과 소아 당뇨환자에서 검진 시 요 알부민을
측정해야 합니다.
7. 만성 신질환 환자에서 단백뇨의 관찰은 요 알부민/크레아티닌 비를
사용해야 하나 요알부민/크레아티닌 비가 높을 경우 (> 500-1000mg/dl)
요 단백/크레아티닌 비를 사용할 수 있습니다.
