1. 당뇨병성 콩팥병이란?
당뇨병으로 인하여 콩팥이 손상되는 합병증을 당뇨병성 콩팥병이라 하며,
당뇨병을 오래 앓은 환자에서 흔히 발생하는 것으로, 제 때 치료를 하지 않은
경우 콩팥기능이 완전히 망가져 투석이나 콩팥 이식이 필요할 수 있습니다.

① 당이 잘 조절되지 않아 혈당이 높을 때
② 과체중 혹은 비만
③ 고혈압
④ 흡연
⑤ 콩팥질환, 고혈압, 인슐린 저항성의 가족력
⑥ 단백뇨 및 미세알부민뇨
3. 당뇨병성 콩팥병의 증상은?
당뇨병성 콩팥병은 대부분 증상이 없다가 말기에 이르러서 증상이 나타남니다. 증
상이 나타나면 치료가 늦는 경우가 대부분입니다.
① 가장 흔한 증상은 고혈압과 부종이며, 그 외 콩팥 동맥 경화증과
단백뇨(콩팥 증후군)증상이 나타남니다.
② 거품이 있는 소변, 부종으로 인한 체중증가
③ 식욕부진, 구역, 몸살기운, 피로, 두통, 잦은 딸꾹질, 전신 가려움증 등
④ 당뇨병 망막증의 시간에 지나면서 악화
⑤ 당뇨병으로 인한 다른 합병증의 비율 증가
4. 당뇨병성 콩팥병의 진단은?
콩팥 기능검사로
① 미세알부민뇨 검사: 콩팥 합병증의 초기에 평가하는 되 매우 유용한 검사로
제1형 당뇨병환자는 당뇨병이 진단된 후 5년이 지나면 매년 1회 정기적으로
소변검사를 합니다
제2형 당뇨병 환자는 진단받은 시점부터 매년 1회 정기적으로 소변 검사를
합니다.
② 혈청 크레아티닌을 매년 측정해야 합니다. 혈청크레아티닌 농도만으로
신장기능을 평가하는 것보다는 사구체여과율을 계산하여 만성신장질환
의 병기를 결정해야 합니다

5. 당뇨병성 콩팥병은 어떻게 치료해야 하나?
당뇨병성 콩팥병의 치료는 혈당조절과 혈압관리가 핵심입니다.
① 혈당조절: 대부분 환자의 혈당은 당화혈색소 7.0% 이하지만 목표치는
개인마다 다를 수 있습니다.
② 혈압관리: 당뇨환자의 고혈압 치료 목표는 130/80mmHg이지만
단백뇨가 있는 경우는 125/75mmHg까지 더 엄격히 조절하기도 합니다.
혈압약은 ACE 억제제 또는 ARB를 주로 사용합니다.
③ 식이조절
단백질 섭취제한 - 단백뇨가 있는 경우 콩팥기능을 보존하기 위하여
단백질섭취를 일부 제한하는 것이 좋습니다.
지질 조절: 지질 조절도 같이 해 주는 것이 좋습니다.
6. 주의 사항
당뇨병성 콩팥병을 가진 환자는 통팥에 손상을 줄 수 있는
방사선 조영제, 비스테로이드성 항염증제, COX-2 억제제 등의
약물 사용을 피해야 합니다.
임신을 계획 중인 경우는 반드시 주치의와 상담해야 합니다.
7. 건강유지를 위한 조언
① 섭취하는 나트륨(소금)의 양을 제한해야 합니다
② 흡연자는 반드시 금연해야 합니다.
➂ 과체중이거나 비만 환자는 체중을 줄이고 적절한 체중을 유지합니다.
