① 항암치료 시작 2주 전, 항암치료 중에는 다음 치료 2주 전에
접종합니다. 생백신은 접종금기입니다.
② 항암치료 시작 2주 전(가능하면 4-6주전)
또는 항암치료 종결 3개월 후 에 접종합니다.
③ 항암치료 시에는 시작 2주 전(가능하면 4-6주 전) 또는 항암치료 종결
3개월 후, 비장 절제술 시행 2주전(2주전에 접종 못했을 경우에는
수술 2주 후)에 접종합니다.
④ 생백신(MMR, 수두, 대상포진 등)은 접종 금기입니다.
생백신 접종 이 필요한 경우 항암치료 종결 3개월 후에 접종을 고려해
야 합니다.
⑤ 가능한 한 이식 전에 면역상태를 확인하여 접종 계획을 수립하고,
통상적으로 이식 후 6개월이 경과하면 면역저하의 정도가 안정되므로
접종을 시작할 수 있습니다.
⑥ 항체 음성인 환자는 이식 후보다는 이식 전에 예방접종을 받도록
권장합니다.
⑦ 인플루엔자 생백신은 이식 전 후 모두 접종 금기입니다.
수두백신은 접종 후 적어도 4주 이상의 간격을 두고 이식을 하도록
권장하며, 이식 후에는 접종금기입니다.
⑧ 조혈모세포 이식 수혜자는 B형간염, 홍역, 파상풍, 디프테리아,
폴리오 등에 대한 정기적인(4-5년마다)항체검사를 권장해야 합니다.
BCG, 폴리오 생백신, 인플루엔자 생백신은 접종금기입니다.
⑨ 생백신은 권장하지 않으며,
인플루엔자 유행 시기에 백신을 접종하지 않은 경우, 이식 4개월이
경과하였으면 즉시 접종합니다.
⑩ 접종시작 시기에 대해 논란은 있으나, 일반적으로 이식 후 3-6개월 후
접종합니다.
⑪ 이식 후 6-12개월에 접종합니다.
⑫ 생백신(MMR, 수두, 대상 포진 등)은 접종금기입니다.
단 접종이 필요한 환자의 경우, 이식 후 2년이 지난 후에 접종하도록
하고, 만성 이식 편대 숙주반응이 있거나 면역저하상태가 지속될 경우
에는 접종하지 않습니다.
대상포진 백신은 바이러스 역가가 높아 접종금기입니다.
⑬ 일반적인 백신 투여량이 2배인 2.0mL을 투여해야 합니다.
⑭ HIV 감염의 CD4+ T 림프구수가 200/㎕ 미만인 경우에는
생백신(MMR, 수두, 대상포진 등)접종 금기입니다.
⑮ 예방접종은 코르티코스테로이드, DMARD(disease-modifying anti-
rheumatic drugs), TNF-α차단제 투여시에도 접종 가능하며,
Riuximab 사용 4주 전에 접종하도록 합니다.
생백신 접종은 면역억제제 사용 종료 3개월 이상 경과 후에 고려해야
합니다.
⑯ 불활성 생백신으로 접종합니다.
⑰ Methotrexate 또는 Rituximab 사용 시 폐렴구균 예방접종에 대한
면역반응이 매우 감소하므로, 투약 4주 전 또는 투약 종료 후 3-6개월
이상 경과한 뒤에 접종해야 합니다.
⑱ 생백신(MMR,수두, 대상포진 등)은 접종금기이나 접종이 필요한 경우,
면역 억제제 투약 시작 4주 전 접종 또는 투약 종료 후 3개월 이상
경과 후에 접종합니다.
