Ⅰ. 결핵의 진단
1. 결핵의 증상
① 뚜렷한 원인 없이 2-3주 이상 기침 등의 호흡기 증상이 있으면 결핵을
의심하고 이에 대한 검사를 시행하여야 합니다.
② 임상 소견상 결핵이 의심되면 결핵의 과거력, 결핵환자의 접촉 여부에
대하여 물어보아야 합니다.
2. 흉부X선 검사
① 흉부X선 검사는 결핵의 유용한 진단방법이지만 흉부X선 단독으로 결핵을
진단하지 않습니다.
② 흉부X선 소견상 결핵이 의심되는 병변이 있으면 가능한 과거에 시행한
흉부X선 사진과 비교하여 보아야 합니다.
③ 흉부X선 검사에서 결핵이 의심되면 객담결핵균 검사를 실시하여 결핵을
확진하도록 노력해야 합니다.
3. 검사실 진단
⑴ 항산균 도말 검사
① 폐결핵이 의심되는 환자는 객담을 최소한 2회, 가능한 3회 채취하여
항산균 도말 및 배양검사를 시행하여야 합니다.
② 항산균 도말검사는 가능한 빨리 시행하고, 도말 양성 결과는 24시간
이내에 통보하여야 합니다.
⑵ 항산균 배양검사
① 결핵균검사를 위해 의뢰된 검체는 항산균 도말 및 배양검사를 동시에
시행하여야 합니다.
② 검체는 고체배지와 액체배지에 각각 접종하여야 합니다.
③ 항산균 증식이 확인되면 신속하게 결핵균과 비결핵 항산균 감별검사를
시행하여 결과를 통보하여야 합니다.
⑶ 결핵균 핵산증폭검사
- 결핵균 핵산증폭검사는 결핵이 의심될 때 도말 및 배양 검사와 함께
1회에 한해 시행을 권고합니다.
⑷ 약제 감수성검사
① 전통적인 방법
- 모든 결핵환자의 첫 배양분리균주에 대해 항결핵제에 대한 약제
감수성검사를 실시하여야 합니다.
- 3개월 이상 치료에도 배양 양성이거나 임상적으로 치료실패가 의심되는
경우에 약제 감수성검사를 다시 시행하여야 한다.
- 신속내성검사, Xpert MTB/RIF 혹은 액체배지 감수성검사에서 약제
내성이 검출된 경우 주요 일차, 이차 항결핵제에 대한 감수성검사를
시행하여야 합니다.
② 신속내성검사
- 재치료 등 다제내성 결핵이 의심되는 경우 도말 양성 검체 혹은 배양된
결핵균주를 대상으로 리팜핀 또는 리팜핀과 이소니아지드에 대한 신속
내성검사를 시행하는 것을 권고합니다.
⑸ Xpert MTB/RIF
① 재치료와 같이 다제내성 결핵의 가능성이 높은 경우 초기진단 검사로
Xpert MTB/RIF를 시행할 수 있습니다.
② 중증결핵이나 인체면역결핍바이러스(human immunodeficiency virus, HIV)
감염인에서 발생한 결핵처럼 약제내성여부를 신속하게 확인해야 하는 경우
Xpert MTB/RIF 검사를 시행할 수 있습니다.
③ 리팜핀 내성 가능성이 낮은 상황에서 내성으로 나오면 다른 감수성 검사법
으로 확인합니다.
4. 조직학적 진단
- 결핵진단을 위하여 조직검사를 시행할 경우 조직검체에 대해 항산균
배양검사와 결핵균 핵산증폭검사를 시행하는 것을 권고합니다.
5. 기타 검사
⑴ 흉부 전산화 단층촬영
① 도말음성 폐결핵의 경우 흉부 X선 검사로 활동성 여부를 판단하기 어려울
때 흉부 전산화 단층촬영을 고려해야 합니다.
② 결핵과 다른 원인질환의 감별이 어려울 경우 흉부 전산화 단층촬영을
고려해야합니다.
⑵ 면역학적 진단(결핵감염검사)
① 결핵감염검사인 투베르쿨린 검사와 인터페론감마 분비검사는 활동성
결핵과 결핵감염을 감별할 수 없으므로 활동성 결핵의 진단을 위해
사용하는 것은 권고하지 않습니다.
② 결핵균 검사가 음성이고 폐외 결핵과 같이 임상적으로 결핵이 의심되지만
진단이 어려운 경우 결핵감염검사가 활동성 결핵의 진단을 위해 보조적으로
사용될 수 있습니다.
6. 폐결핵의 진단과정
⑴ 도말 양성이면서 결핵균 핵산증폭검사 양성인 경우
① 폐결핵으로 진단고 결핵 치료와 경과를 관찰합니다.
② 재치료 등 다제내성 결핵이 의심되는 경우 도말양성 객담을 대상으로
리팜핀 또는 리팜핀과 이소니아지드에 대한 신속내성검사를 시행하거나
초기진단검사로 Xpert MTB/RIF검사를 시행합니다.
③ 추후 배양검사를 확인하여 결핵균이 확인되면 약제감수성검사를 시행합니다.
⑵ 도말 양성이면서 결핵균 핵산증폭검사 음성인 경우
폐결핵이 아닌 비결핵항산균 폐질환으로 잠정진단하고 추후 비결핵 항산균
배양을 확인하고 균동정검사를 시행하여야 합니다.
⑶ 도말 음성이면서 결핵균 핵산증폭검사 양성인 경우
① 폐결핵로 진단하고 결핵치료 후 경과를 관찰합니다.
② 추후 배양 검사결과를 확인하여 결핵균이 확인되면 약제감수성검사를
시행합니다. 재치료등 다제내성 결핵이 의심되는 경우 배양된 결핵균주를
대상으로 리팜핀 또는 리팜핀과 이소니아지드에 대한 신속내성검사를
시행합니다.
⑷ 도말 음성이면서 결핵균 핵산증폭검사 음성인 경우
환자의 임상증상 또는 방사선학적 소견에서 폐결핵이 의심되지만,
객담 도말검사와 결핵균 핵산증폭검사가 음성이면서, 항결핵 효과가 없는
광범위 항생제 치료에 반응이 없고 진료의사가 전기간의 결핵치료를
시행하기로 결정한 경우를 도말음성 폐결핵이라고 정의합니다.
