1. 혈압측정

혈압은 상황에 따라 변화가 크기 때문에 여러 번 측정해야 합니다.

고혈압의 진단, 치료및 예후 평가에 가장 첫 번째 중요한 것은 정확한

혈압측정입니다.

1) 진료실의 혈압측정

올바른 혈압 측정방법

● 혈압 측정 전 최소 5분 동안 안정해야 합니다.

● 1-2분 시간 간격을 두고 적어도 2번 이상 혈압을 측정해야 합니다.

● 팔의 80% 이상을 감을 수 있는 낭대(bladder)를 사용해야 합니다.

(표준길이 12-13cm X 너비 35cm)

● 환자의 심장높이에서 혈압측정 띠를 둬야 합니다.

● Korotkoff음의 Ⅰ과Ⅴ를 각각 수축기 및 확장기혈압으로 측정합니

다.

☎ 코르트코프음 Ⅰ은 처음 소리가 들릴 때 측정하며

코르트코프음 Ⅴ 은 소리가 들리지 않을 때를 의미합니다.

● 처음에는 양팔에서 혈압을 측정하고 이후에는 높은 쪽에서 측정합니

다.

● 노인, 당뇨병, 기립성저혈압이 있는 경우에는 일어선 후 1분, 5분에

기립혈압(일어나서)을 측정합니다.

2) 가정 혈압 측정

집에서 혈압을 측정하면 치료약물에 대한 효과 및 백의 고혈압의 평가

에 도움이 되고 환자의 순응도를 높이는 이점이 있습니다.

가정혈압 측정이 표적장기 손상의 정도 및 예후에 더 좋은 상관관계를

가진다는 보고가 있습니다.

혈압 측정은 혈압이 안정된경우에는

주3회, 하루 2회가 측정하는 것이 좋습니다.

아침에는 기상 후 1시간 내, 소변 보고난 이후에,

저녁에는 취침 전 안정 후 측정하고

혈압과 맥박수를 동시에 기록해야 합니다.

집에서 잰 혈압은 병원에서 측정한 것보다 평균 12/7mmHg가 낮습니다.

가정혈압의 평균이 135/85mmHg 보다 높으면 고혈압으로 간주합니다.

☎ 표적장기란 혈압이 높을 때 우리 몸에 이상이 일어나는 장기를 말하며

뇌, 눈, 심장, 콩팥 및 기타 말초혈관을 말합니다.

☎ 백의 고혈압이란 흰 가운만 보면 혈압이 높게 나오는 것을 말합니다.

즉 의사가 혈압을 재었을 때 환자가 불안하여 일시적으로 혈압이 높게

나타나는 경우를 말합니다.

3) 24시간 활동 중 혈압측정

낮 시간 활동할 때와 수면 중의 혈압에 대한 정보를 알 수 있습니다.

야간혈압은 주간의 혈압보다 약 10%가 낮습니다.

활동중 혈압이 진료실 혈압에 비해 좌심실비대, 단백뇨, 망막병증

등의 표적장기 손상을 예측하는데 더 좋습니다.

고혈압 환자중에서 야간에 혈압이 감소(10%)가 없는(non-dipper)예

에서 있는 예(dipper)보다 심근경색, 뇌졸중 같은 심혈관사고의 위험

이 3배 더 높습니다.

24시간 활동 중 혈압을 측정하는 경우는

⑴ 벡의고혈압이 의심될 때

⑵ 고혈압약을 복용하여도 떨어지지 않을 때

⑶ 간헐적인 고혈압이 있을 때

⑷ 자율신경 장애가 있을 때 등입니다.
