Ⅳ. 혈당조절의 감시

1. 자가혈당 측정

① 최소한 공복과 식후 2시간 혈당(식사 개시 후 2시간 째)를 포함하여

측정합니다.

② 측정의 횟수는 환자의 혈당조절 정도에 따라 다르지만 임상영양치료

운동요법, 경구 혈당강하제 치료, 2회 이내의 인슐린 치료를 하는

제2형 당뇨병 환자에서 매일 최소 1회 이상의 자가혈당 측정을

하여야 하며, 다회인슐린 치료를 치료를 필요로 하는 제1형 당뇨병

및 제2형 당뇨병 환자에서는 매일 최소 3회이상의 자가 혈당 측정을

하는 것이 좋습니다.

2. 당화혈색소 측정

① 일반적으로 당뇨병 환자에서 1년에 4회 이상 당화혈색소를 측정합니

다.

② 당뇨치료가 잘 되는지 평가는 당화혈색소를 기준으로 합니다.

Ⅴ 혈당조절 약제

1. 경구혈당 강하제

① 고혈당의 정도는 당화혈색소로 평가하는 것이 좋으며, 당화혈색소의

목표는 7.0% 미만으로 합니다.

② 만성질환 3단계 이상의 경우 저혈당의 위험이 증가 될 수 있습니다.

③ 신기능이 감소된 만성신질환 환자에게 경구혈당강하제를 사용할 때

는 선호 및 금지되는 약제를 반드시 구분하여 사용해야 합니다.

2.인슐린

① 당화혈색소가 10.5% 이상이거나 고혈당 증상이 동반되는 경우에는

처음부터 인슐린 치료를 고려 해야 합니다.

② 경구혈당강하제 병합요법에 실패 시 조기에 인슐린 치료를 고려해야

합니다.

③ 신기능이 감소된 만성 신질환의 경우 인슐린 치료를 적극적으로

제안합니다.

☎ 1. 약물 축적 위험이 적은 약제를 우선적으로 선택합니다.

① sulfonylurea계: glipizide 나 gliclazide 이 좋으며

glimepride는 저용량으로 사용 할 수 있습니다.

② 비 sulfonylurea계

Meglitinide: repaglinide가 안전한 약제로 알려져 있습니다.

biguamide: 젖산 위험성이 있어 사용을 피합니다.

ɑ-glucoside inhibitor: 체내 흡수가 증가되므로 사용에 주의

해야 합니다.

thiazolidinedione: 기왕의 울혈성 심부전 악화의 가능성이

있으므로 NYHA분류 3.4는 사용하지 않는 것이 좋습니다.

장기사용 여성에서 골절위험이 증가하므로 조심하는 것이

좋습니다.

2. 경구 혈당약제의 특성에 따라 사용에 제한이 많으므로 인슐린의 적극

적인 사용이 필요합니다.
