Ⅲ. 비알코올 지방간질환의 진단

1. 비알코올 지방간염의 진단 및 선별검사

비알코올 지방간질환의 진단을 위해서는 간 내 지방변화의

확인뿐 아니라 다른 원인의 간질환이 아니라는 것이 중요합니다.

즉 간기능검사에서 AST 또는 ALT(GPT 또는 GOT)의 상승이 발견된

경우 B형 및 C형 간염, 알코올 간 질환, 약물유발 간염,

자가면역 간질환, 윌슨병 등의 다른 간질환과 근육질환 등의 감별을

위한 병력청취 및 검사가 필요하며

일차적으로 복부 초음파 검사를 시행하여 간의 지방증을 확인 하여야

합니다.

영상학적으로 우연히 간의 지방 변화가 발견된 경우 대사증후군,

제2형 당뇨병 및 관상동맥질환 등에 대한 검사를 고려해 볼 수 있

습니다.

2. 비알코올 지방간염 및 진행된 섬유화에 대한 비침습적 검사법

1) 영상의학 검사

초음파검사, CT, MRI, Transient elastography(MRS)는 간내 지방량의

평가에 도움이 될 수 있으나, 비알코올 지방간과 지방감염의 감별에는

도움이 되지 안습니다.

2) Transient elastography(TE)

간내 섬유화 평가에 있어서 높은 민감도와 측이도를 보여주고 있지만

비만환자에서는 정확성이 떨어집니다.

3) Magnetic resonance elastography(MRE)

비알코올 지방간과 비알코올 지방간염 및 섬유화를 감별하는데

유용하나 검사비가 고가이며 이 장비는 모든 의료기관에 다 있지

않습니다.

4) 생화학적 패널

⑴ AST/ALT비 - 가장 일반적이고 쉬운 검사이나 간조직내의 섬유화나

염증정도를 정확히 반영하지 못하고 미알코올 지방간과 지방간염을

감별할 수 없고 치료에 따른 효과도 반영하지 못합니다.

⑵ NAFLD Fibrosis Score(NFS) - 6개의 표식자(연령,체질량지수(BMI),

당뇨병/내당능장애의 유무, 혈소판 수, 알부민, AST 와 ALT(SGPT와

GOT)로 구성되어 있고 웹사이트( http://nafldscore.com)에서 쉽게

계산할 수 있습니다.

⑶ Enhanced Liver Fibrosis(ELF) - 현재 유럽에서 간섬유화 평가방법

으로 연구가 진행되고 있으며 간섬유화와 괌련된 3개의 기질단백으로

구성됭 있습니다.

☎ ① 초음파검사, CT, MRI, Transient elastography(MRS는 간내 지방

량의 평가에 도움이 될 수 있으나, 비알코올 지방간과 지방감염의

감별에는 도움이 되지 않습니다.

② NAFLD Fibrosis Score(NFS), Transient elastography(TE) 및

Magnetic resonance elastography는 비알코올 지방간질환에서

진행된 간섬유화의 평가에 도움이 될 수 있습니다.

◉ NAFLD Fibrosis Score(NFS)가 가장 많이 검증되어있고 비교적

간단히 시행할 수 있습니다.

3. 간조직검사

비알코올 지방간질환에서 비알코올 지방간염 또는 진행된 감 섬유화가

의심되는 경우에는 간 조직검사를 생각해 봐야 합니다.

비알코올 지방간질환에이 의심되는 환자에서 다른 만성 간질환의 동반을

배제할 수 없을 때에는 간 조직 검사를 고려해 봐야 합니다.
