갑상선 기능항진증은 갑상선에서 갑상선 호르몬이 과다하게 생선되어
갑상선중독증이 나타나는 상태이며, 갑상선 중독증의 흔한 원인 질환은
그레이브스병(82.7%), 무통성갑상선염(13.3%), 아급성갑상선염(3.5%),
중독성결절(0.5%)순입니다.
그레이브스병은 TSH수용체에 대한 자가항체가 갑상선을 자극하여
미만성 갑상선종 및 갑상선 기능항진증을 초래하는 자가면역 질환으로서
안병증(ophthalmpophathy), 피부병증(dermophathy)등을 동반할 수
있습니다.
1. 갑상선 기능항진증의 진단
1) 중등도의 평가
특히 심혈관계, 신경근육계의 합병증이 있는지 여부를 정확히
평가하여야 합니다.
특히 고령의 환자는 갑상선중독증의 증상 및 징후가 잘 나타나지
않으므로 특히 심장기능의 평가가 필요한 경우가 많습니다.
2) 혈액검사
혈청TSH 와 유리 T4를 같이 측정하는 것이 좋습니다.
갑상선호르몬 과잉의 상태를 평가할 때는 혈청 TSH가 더 예민합니다.
⑴ 현성 갑상선 기능항진증 - TSH는 측정한계치 이하로 억제되고
혈청유리 T4와 T3가 가 모두 증가됩니다.
⑵ 경미한 갑상선 항진증 - 유리T4는 정상이고, T3만 상승되며
TSH는 억제되는데 이것을 T3-toxicisis라고 합니다.
이것은 갑상선기능항진증의 초기 또는 자율기능성 갑상선결절에서
주로 나타납니다.
⑶ 불현성 갑상성기능 항진증 - TSH는 정상보다 낮으나 혈청T3,
유리 T3, 유리 T4는 정상입니다.
☎ TSH를 분비하는 뇌하수체종양 또는 갑상선호르몬 저항증후군의 경우를
제외하고는 혈청TSH가 정상이면 환자는 정상 갑상선기능이라 할 수
있습니다.
① TSH가 정상이면서 총 T4치가 높은 경우를 euthyroid
hyperthyroxinemia라고 하는데 이 경우는 갑상선기능항진증이
아니고 대개 T4에 결합하는단백의 농도 변화에 기인합니다.
즉 갑상선호르몬 결합단백의 유전적 변이, 임신 또는
여성호르몬제 투여, 간염, 다양한 약물 등에 의해 나타날 수
있고 T4에서 T3로 전환이 억제되는 상황에서도 나타날 수
있으며 더 이상 검사는 필요하지 않습니다.
② 그러나 euthyroidhypothyroxinemia가 아니라면 TSH에 의한
갑상선기능 항진증의 가능성을 고려하여 MRI검사에서 뇌하수체
종양이 있고 TSH α-subunit가 증가되 있으면 TSH뇌하수체
종양입니다.
③ 가족력이 있고 T3-수용체 유전자의 돌연변이가 있으면 갑상선
호르몬 저항증후군으로 진단합니다.
3) 병인
갑상선중독증의 임상상을 보이는 환자에서 TSH수용체 검사를 시행하여
그레이브스병을 진단할 수 있습니다.
TSH수용항체 검사를 시행할 수 없는 경우에는 갑상선 섭취률 검사나
갑상선 스캔을 이용하여 갑상선 기능항진증을 진단할 수 있으며
초음파검사(Doppler검사)도 감별진단에 도움이 됩니다.
